Appointments scheduled in your home Monday through Friday, with evenings and Saturday appointments by request.
For optimal health outcomes we recommend a minimum schedule of every 3 month appointments. More frequent access will be arranged according to specific conditions. Sick care appointments and telemedicine conferencing are always available for our members.
Two fees apply: 1) Affordable concierge fee payable monthly or once per year. 2) Visit fees which are reimbursable by Medicare Part B when patient meets home bound criteria.
Patients with other insurance plans or those not meeting strict home bound criteria should also inquire.
