In standard practice, patients must travel to a doctors office. There is time required for check in, time spent with the nurse then about 15 minutes with the doctor. The journey back home is next followed by pharmacy visits, maybe lab appointments and so forth. As we age, many patients are challenged by health conditions that make these visits challenging and exhausting. “I’m too sick to go to the doctor!” some say. Enter the house calls doctor, taking a cue from the past delivering a personalized visit without the rush, without the hassle. A house calls doctor making regular visits can make a positive impact on health by identifying problems early, personally reviewing medications and supplements, knowing more detailed information about a patient and eliminating the delays and hazards of traveling to a medical office. The benefit is proven. Medicare’s independence at home study revealed that this care model saves money and prolongs quality of life. We interviewed a local doctor, Caron Houston, MD, about this new practice model coming soon to Carmichael, California.
WP: Very few doctors make house calls these days, why have you chosen this path?
CH: I have always loved geriatric practice, this opportunity allows me to continue this work in an even better model. At a house call, I get to know so much more about my patient, I see the caregiver team, the home environment and get to spend more time.
WP: What about the health care team, if you are not in an office, how are other services provided?
CH: Home care services are available and many are covered by insurance. nurses, physical therapists, social workers and wound specialists can be referred for specific conditions. A local team will come to the home for blood draws and several pharmacies make deliveries. Medical equipment companies will rent or provide devices with a doctor’s order and agencies have caregiver staffing to help families in need.
WP: Are house calls affordable?
CH: Medicare does cover a house call visit from a doctor or other medical provider but travel costs are not included. Other services such as telemedicine and after hours care are covered by a concierge fee. Studies have shown dramatic health benefit and cost savings for patients.
WP: Which patients are eligible for house calls care?
CH: A patient who is home bound for health reasons is appropriate for care. The HCC team focuses on frail seniors who reside in the community or assisted living facilities. The house call care team’s goal is optimize patients wellness and avoid hospitalization or nursing home placement when possible.
